Hospital Discharge To Home Care

My mother (81) received a care package on discharge from hospital following a falls incident which resulted in a fractured leg.  Mam has other health issues and we were very worried as to how she would manage at home on her own.

I had heard from a friend about Applewood Homecare and as they are on the list of HSE Approved Providers I rang them straightaway when the ward nurse first said about discharge.

I spoke with Kate, their Care Manager and she really reassured me with the detail of the questions she asked and we arranged to meet in my mam’s house the next day.

I was very impressed with how she explained the service and the information she left with me to consider.

We had a bit of a lead-in before the discharge and Applewood provided us in advance with a named roster of carers to help mam get up and dressed etc in the morning, someone in during the day to make sure she has a lunch, takes her medicines and did the exercise which the OT gave her on discharge, and later on in the evening to do the bed-time routine.

We are delighted that everyone in the team were introduced to mam formally to put her at ease and that the consistency promised is being delivered.

The service is a great support to us and everyone is very caring and professional and attentive.  Couldn’t fault them and they’ve been with mam now over a year.
